Documents required to obtain a Ukrainian passport at age 14:

  • Birth certificate (if available);
  • Ukrainian passports of both parents (if available);
  • Certificate of Ukrainian citizenship (in cases of acquiring citizenship under certain conditions, such as: one parent is a foreign national, the child was born in Ukraine to two foreign nationals, or other legal grounds under Ukrainian legislation or international treaties);
  • Certificate of taxpayer identification number or official notice of refusal due to religious beliefs (for individuals who refused to obtain the number for religious reasons);
  • Certificate of registered place of residence (if available).

Documents required to obtain a Ukrainian passport under other conditions:

Obtaining a passport in case of loss, theft, or damage:

  • Certificate of registration/deregistration of place of residence (if available);
  • For individuals arriving from territories temporarily occupied by the Russian Federation: additional photo ID documents are required;
  • For internally displaced persons (IDPs): certificate of IDP registration;
  • For homeless individuals: certificate of registration as a homeless person;
  • Damaged passport.

Passport replacement due to change of: full name, errors in date of birth or personal details, change in parents’ citizenship, gender, or other reasons:

  • Marriage or divorce certificate, document confirming name change, court decision (if applicable), new birth certificate in case of gender change.

Replacement of 1994-type passport with a biometric ID card (upon request or at age 25 or 45):

  • Taxpayer identification number certificate or notice of refusal (for individuals who declined it for religious reasons);
  • Certificate of residence registration (if available).

Obtaining a passport after refusal by the State Migration Service (SMS):

  • Official decision on passport denial (if the refusal was issued in writing).
